Application Process

The SHRS Dietetic Internship is accredited by the Commission on Accreditation of Dietetic Education (CADE). We participate in the D&D Digital Systems match process. Sixteen (16) applicants will be accepted for the 2009-2010 class.

Application Due Date

Application materials must be postmarked by February 16, 2010. Applicants will be notified of acceptance or rejection of application by letter. Selected applicants must send a certified letter to the Program Director to confirm the acceptance or rejection of the appointment.

Internship Entrance Date

Mid-August, 2010.

Enclosures To Complete Application

Applications and the listed enclosures are to be sent to Jacquelynn O’Palka, Ph.D.,Chair, Nutrition and Dietetics Department, Coleman Hall Room 224, 1140 West Michigan Street, Indianapolis, IN 46202. Applications will not be evaluated without ALL required materials listed below: (All materials except the letters of reference should be mailed together in the same envelope.)

  1. Application form filled in with a typewriter or printed in ink.
  2. Original hand-written questionnaire (included in packet).
  3. GRE scores and OFFICIAL transcript(s) of ALL credit and grades from ALL colleges and universities attended. Be sure last term and transfer grades are included.
  4. Official Didactic Program Director's Verification of Completion (or Declaration of Intent to Complete) of The American Dietetic Association academic requirements in the General Area of Specialization or Standards of Education with original signatures. Photocopies are not acceptable.
  5. List of work experience. See application form.
  6. List of courses and grades. See application form.
  7. Reference letters from three or more professional persons qualified to give pertinent information as to character, ability, professional potential and personality of the applicant. The references should include a professor in food and nutrition, professor in food service systems management, and a supervisor of work experience (photocopies of references will be accepted). Please use the standardized recommendation form if available from your Didactic Program Director. Reference letters must be mailed directly to the program.

Selection Criteria

Applicant ranking by our Selection Committee is based on quality of questionnaire, references, work experience, GPA and compliance to application requirements.Preference will be given to applicants with an Academic GPA at or above 3.0 (A = 4.) Overall. Minimum cumulative grade point average which will be considered is 2.9 (A = 4). Minimum grade point average for course work in the major is 3.0 (A = 4.0). All required science grades must be C or better.

Disposition of Application Materials

Unsuccessful applications will be destroyed at the close of the selection process.
